Amendments, Addenda, Etc.

The following contracts and forms have been approved by Contract Administration and are considered “standard” for University purposes. In general, standard University contracts under $50,000 do not require further approval by Contract Administration and may be processed within the originating department/college. Subject to limited exceptions set forth in University policies, all other agreements typically need to be reviewed and approved by Contract Administration.

Because we revise our standard contract forms periodically based on changes in law or University policy, we strongly recommend that you visit this site each time you need a contract form - as opposed to storing and reusing forms on your computer/network, which could result in your use of an outdated or otherwise inappropriate document.

Alternative Dispute Resolution Clause (ADR) (OGC-S-1999-24)- addendum that incorporates the dispute resolution process under Texas Government Code Chapter 2260

Child Support Certification (OGC-S-1999-25)- addendum in which a vendor certifies compliance with Texas Family Code Section 231.006; for contracts paid for with state funds

Foreign National Information Addendum (OGC-S-2006-15)- addendum used for contract services being performed by a foreign nationals

OCA Addendum (OGC-S-2012-13) - addendum used for activities involving children

OCA Addendum for Facility License Agreement (OGC-S-2015-09) - addendum used for activities involving children at Campus Recreation

Standard Contract Addendum (OGC-S-2009-01) - used to attach to the vendor contracts that are under $50,000.  If the vendor agrees to sign this Standard Addendum along with their contract, then the contract may be processed as if the contract was a University standard contract.

Standard Amendment (OGC-S-2010-08)– used to amend an existing agreement

Student Background Check Addendum (OGC-S-2006-03)- addendum to contracts that address student drug testing, health care and criminal background checks
